Students may email completed forms and required documents to CPR and Aquatics LLC to m.aikens@cpraq.com.

Students must submit their MDHHS vaccination record to CPR and Aquatics LLC for verification the student has had all required childhood vaccines. Required vaccinations are:
Students must also have a TB test done. If the TB test is positive, the student must consult with their physician for a chest x-ray and provide documentation from their physician that they are clear of tuberculosis.
All students must sign a consent form allowing CPR and Aquatics LLC to perform a criminal background check, to ensure the student is eligible for EMS courses.
Students must complete the last page of the EMS Student Syllabus and return it to CPR and Aquatics LLC.
Students must also submit authorization from a physician that state the student is cleared to use a respirator as necessary.
Students must provide proof that they have an active health insurance policy.
Students must submit a copy of a valid motor vehicle operators license. A drivers license, chauffeurs license, or other advanced motor vehicle operators license is acceptable. This license must not be expired.
